On Wednesday evening, Evolutions Medical Spa and Santa Barbara cosmetic surgeon Dr. Terry Perkins hosted an interactive and educational event about Tightening and Lightening the skin. Both tightening and lightening can be done at Evolutions through various services, without invasive surgery, and will yield noticeable results!

Part I: SKIN TIGHTENING - Facial Rejuvenation

What is skin laxity?

Skin laxity is a result of damage and loss of collagen and elastin - the structural components of the dermis. Collagen provides the rigidity to connective tissues and elastin allows them stretch. As we age collagen loses elasticity and our skin sags. Sun damage also plays a role in aging the skin as well.

How can we treat skin laxity?

There are several options including:

Surgical options such as traditional surgery (face lifts) or deep peels (chemabrasion).
Non-surgical treatments include light-based (FotoFacial), radiofrequency (Sublative Rejuvenation, ReFirme), and focused ultrasound.

How does treating skin laxity non-surgically work?

It has been found that sustained heating of the dermis works to treat skin laxity. The immediate effect is collagen contraction and long term stimulation of collagen remodeling.

Why do you recommend skin tightening treatments in a series?

Laser and light-based treatments are generally done in a series because the energy necessary to cause dramatic change in the skin (which cause injury and then recovery, which is what grows new skin) cannot be accomplished in a single treatment, instead it is the cumulative effects of multiple treatments that yield the best results. If, after a few treatments, a client sees the results they are looking for, they can save the remaining treatments to be used as "maintenance" at a later date.

Part II: SKIN LIGHTENING - Improve Excess Pigmentation

What is pigmentation?

Pigmentation is created by melanin in the skin. Melanin is produced by melanocytes. Different skin types have different levels of melanocyte activity - therefore different skin colors occur within people.

Why does the body produce melanin?

Normal production of melanin is based on skin type. Melanin levels rise in response to sun exposure (such as a sun tan which is equal to a skin injury), trauma, and inflammation. Extra melanin can also be produced due to hormones, birth control pills, and pregnancy.

When do pigmentation problems occur?

If melanocytes are over-stimulate, appearance problems can occur such as:

Sun damage also referred to as age spots, "liver" spots, freckles
Post inflammatory hyperpigmentation can occur with pimples
Melasma - a broad patch of pigmentation - can occur in the dermal or epidermal layers of the skin

Treatment for pigmentation problems

The following highlights products and services that are available at Evolutions to treat a number of pigmentation problems.

Sunscreen: your best defense against pigmentation is to prevent it! Year round, daily use of sunscreen is always recommended!

Home Care: there are two categories of home care products that can help lighten and brighten the skin - bleaching agents and exfoliating agents.

Home care bleaching agents: Obagi Clear, C-Rx Night (with hydroquinone), the newest, fastest lightening agent Elure (read more about Elure in a recent blog post), Revale Illumenesse Brightening Complex, Nia24 products, and retinoids ("Retin-A"). Exfoliating agents assist in increasing cell turnover: products with alpha-hydroxy acids, amino fruit acids and retinoids all help to expedite cell turnover (which slows as we age).

Chemical Peels: the fastest way to exfoliate and increase cell turnover is through a chemical peel. Available in multiple strengths, from light to deep, chemical peels can make a dramatic difference in the skin in a matter of days (somewhere between 5 to 10 days, with the skin continuing to improve thereafter). Vibradermabrasion: this service provides a physical exfoliation that can treat superficial pigment problems (with no downtime). It also is a great add-on service to facials and as a preparation for peels as it will improve the penetration of product or the peel. Vibraderm is superior to the popular microdermabrasion treatment because there is no suction component to it (which can cause broken capillaries).

Light-Based Treatments: these treatments are safe and non-invasive, using intense pulsed light (IPL) and radio frequency technology to diminish imperfections in the skin such as excess pigment as well as "red" areas. The FotoFacial RF is the "gold standard" for treatment of pigmentation and it works deeper than most chemical peels. The FotoFacial is available in a series or in combination with other treatments (FotoPeel, FotoPeel Plus, and FotoFaical Genesis Package).

*NEW* Light-Based Treatment: Evolutions just acquired the brand new Q-Plus C (Q-switched laser) light-based treatment that is able to treat tattoos of all colors and a variety of pigmentation issues.

Sublative Rejuvenation: this fractional surfacing device uses radio frequency energy, instead of light, to treat wrinkles, pigment, acne scarring, skin laxity, tone and texture. There is less downtime and less discomfort that older fractional laser devices (such as the Fraxel) and customizable treatment options are available. Sublative Rejuvenation can be combined with other light-based treatments such as the FotoFacial RF or Total Tightening.

Chemabrasion: this is a surgical procedure that combines a deep peel with dermabrasion. Chemabrasion is a more aggressive option to treat acne scarring and deep wrinkles. The change can be very dramatic. Down time is approximately one week.

Part III: CONCLUSION

The best way to treat sun damage is to prevent it by daily use of sunscreen. When pigment issues arise, there are a number of treatment options available, both at Evolutions and at home. All areas of the body can be treated for pigmentation problems.

Chemical Peels: What They Are and What They Can Do For You

Chemical peels are "the best bang for your buck" because they provide a great exfoliation in a short amount of time that usually results in dramatic changes to the skin (dependent upon the strength of the peel: stronger peels yield more noticeable results). In general, besides the immediate results of smoother skin and removal of environmental damage, chemical peels also help keep you ahead of the aging process.

How Skin Turnover Works

Basic facts about skin turnover helps to understand how chemical peels work. Skin cells are created in the basal layer and move upward. Cells die as they get closer to the skin and these surface cells fall off, to be replaced by new cells. In young people, a normal turnover is about 28 days, but as we age, the rate decreases to 40 or more days. Along with the decrease in skin turnover, the collagen and elastin in the skin decreases, combined with sun damage, and the result is thinner, less plump skin.

What Do Chemical Peels Do?

Chemical peels expedite cell turnover through exfoliation and stimulate new collagen to thicken the dermal layer. New cells replace the exfoliated cells at a faster rate and collagen and elastin production is increased.

The Benefits of a Chemical Peel

In the big picture, regular peels can create healthier, younger-looking skin throughout your lifetime. They can smooth the skin and improve texture and color uniformity. Peels will also:

Lighten by getting rid of superficial damage (age spot, sun spots)
Tighten by increasing collagen and elastin
Brighten by getting rid of the dead cells on the surface that build up (especially when cell turnover slows down with age) and creating plumper looking skin.

Question from the audience: Would peels get rid of freckles?

Answer from Dr. Perkins: Freckles are typically deeper down and peels would not get rid of freckles. If you are only trying to get rid of freckles, a peel would not be the right choice.

Do Chemical Peels Hurt?

The mild peels have a warm, tingly feeling and may create a minor discomfort. For the medium and darker peels, Evolutions has cooling devices to help make a peel more tolerable.

Will Skin Peel Off?

Yes, the superficial layer does peel off but since the peel is a "controlled injury" to the skin it does not produce scarring. Some lighter peels, such as the AFA Peel, creates a micro-exfoliation that may have very minor peeling such as flaky skin, but still gives you the benefit of a peel.

Question from the audience: When do you see peeling?

Answer from Dr. Perkins: In medium to deeper peels, the peeling may begin anywhere from 2 to 5 days post treatment.

Where Can Peels Be Done?

Realistically peels can be done anywhere on the body. Generally it is okay to be more aggressive on the face, but below the jawline you want to be more conservative.

Are Peels Mainly Done for Aging?

No, peels can be done for skin with acne and hyperpigmentation issues.

Are Peels as Effective as Laser Treatments?

Peels and laser treatments do different things. Laser energy can go deeper than peels can. Chemical peels are more reasonable to do cost-wise and are often a good starting point to get rid of superficial damage. For deeper damage, laser treatment may be considered. Note that Evolutions does offer a FotoPeel series that combines a FotoFacial using Intense Pulsed Light (IPL) and RadioFrequency energy to help with blood vessels, brown spots and more. These are alternated with chemical peels to get the maximum benefit from both treatments.

Question from the audience: Do peels help with large pores?

Answer from Dr. Perkins: Not necessarily. Pore are better treated with Laser Genesis that heats the collagen to tighten the pores.

Skin Care for Women in Their 20s from a Santa Barbara Medical Spa

- Skin Care Tips for Women in Your 20s -

Often the early 20s is a transitional period for many young women. For most this means that their hormones have become more balanced and they see a decrease in acne breakouts (although for some acne continues well into the 20s and beyond - r ead more about acne management tips from Santa Barbara cosmetic surgeon Dr. Perkins).

Developing a good skin care regimen is important to do in your 20s because it sets the tone (literally and figuratively!) for the future of your skin. Damage to skin could have started as early as childhood (from excessive exposure to sun and/or sunburns) but may not show up until your 30s and beyond so now is the time to begin a regimen to keep your skin healthy and, hopefully, to correct some of the damage and/or, at the very least keep, it at a minimum.

Make an investment in your skin. Start using the Clarisonic skin care brush twice daily with a mild cleanser dedicated to your skin type (oily, normal or dry). Toss the benzoyl peroxide-based teenage face wash formulas.
Use a broad-spectrum SPF 30 (or higher) sunscreen that provides a physical sunblock. Obagi or TiZo makes sunscreens that contain titanium dioxide and/or zinc oxide that provides a physical barrier to the sun's penetrating UVA and UVB rays. The price tag may be more than you are used to, but a little goes a long way. Prevention of sun damage now means less money spent in your future on trying to erase sun damage! Use daily, year round, and reapply after swimming or excessive perspiration.
Use a retinoid two to five times per week at night. Retinoids (commonly known as the brand name "Retin-A") were traditionally prescribed to fight acne. Additional benefits of retinoids have surfaced such as increasing cell turnover, boosting collagen, stabilizing aging cells and helping to reverse photodamage (in the forms of wrinkles and age spots).
Use an exfoliating wash or serum three to five times per week to remove dry or dead skin cells, increase cell turnover, decrease clogged pores which leaves your skin with a healthy glow.
If your skin is on the oily side, you still need to keep your skin hydrated with an oil-free moisturizer that delivers all the benefits of a regular moisturizer without pore-clogging ingredients. If you're skin is dry, use a regular moisturizer. Apply the moisturizer at night.
And if you are still experience breakouts, consider using a 2% salicyclic acid and/or benzoyl peroxide cream as needed.

There is no better time to start a great skin care regimen than in your 20s. You won't regret it and you will thank yourself in the years to come!

The Five Essential Steps of a Comprehensive Skin Care System

In The News: FDA to Enforce Stricter Labeling for Sunscreens

Santa Barbara cosmetic surgeon Dr. Terry Perkins (and owner of Evolutions Medical Spa Santa Barbara) always reinforces the very important role that sun protection plays whenever people ask him how to prevent wrinkles (or prevent further wrinkles). Being regimented about your sun protection - which includes using a sunscreen that blocks out both UVA and UVB rays and reapplying it after excessive exposure to the sun, swimming or sweating, and using additional protection (such as hats or sun-protective clothing with SPF) - will not only help to prevent wrinkles, premature aging, and hyperpigmentation (darker spots on the skin) but also plays a great role in the prevention of skin cancer.

Dr. Perkins is not alone in advocating that people be diligent about sun protection. In fact, in the news this morning, the US Food and Drug Administration (FDA) is now taking a more active role in how well a sunscreen product protects users against ultraviolet A rays (UVA). UVB rays make people burn, whereas UVA rays penetrate deeper, which can be more damaging in the long run.

According to an article on ABC.com "World News with Diane Sawyer", the FDA will begin to enforce more rigid standards on the manufacturers of sunscreens. Those that promote a "broad spectrum" claim (meaning they protect against UVA and UVB rays) "will have to show that the amount of UVA protection proportionally increases as the SPF level increases" and "the product will also need to be SPF 15 or higher". Additionally, "sunscreens will have to include a drug facts box on the back of the bottle", they no longer will be identified as "sun block" because this implication is inaccurate, and they can no longer claim the product offers more than two hours of protection or "claim the product as water-proof or sweat-proof without indicating the length of time it can provide full protection".

The article also included input from Dr. Mark Abdelmalek, chief of dermatology and ABC News medical contributor who was quoted as saying that "zinc oxide and titanium dioxide are physical sunscreens which are already known to reliably protect against UVA and UVB rays".

What is most unique about our Santa Barbara laser hair removal technique is that we have multiple devices available to treat different hair and skin types which allows us to get better results. With a standard hair reduction treatments, most people see about a 70% reduction in hair growth. About Laser Hair Removal:

Uses light energy to "injure" the hair follicle so it shrinks and has little, or no, hair growth.

Hair grows in cycles so multiple treatments are needed to insure that all the hair in the area gets treated.

Laser Vein Removal and Sclerotherapy Vein Treatments

Sclerotherapy is a highly effective way to treat clusters of red, blue or purple veins (that frequently appear on the thighs, calves and ankles).

Stretch Mark Removal

Santa Barbara Dermal Fillers Restylane and Perlane Review and Recap

Last night at Evolutions Medical Spa Santa Barbara we hosted an event on Dermal Fillers to an at-capacity crowd. On hand was a representative from Medicis, the pharmaceutical company that produces the popular dermal fillers Restylane and Perlane, and also the anti-wrinkle neuro-modulator Dysport.

We have come to realize that many people do not know the difference between dermal fillers and neuro-modulators such as Botox and Dysport. Botox and Dysport get so much press that people often assume that Botox is the end-all solution to turning back the clock and "erasing" wrinkles! This is not necessarily the case for many people.

Before we get into an in-depth look at dermal fillers, we will give you a brief description of Botox / Dsyport and how they work in your face. Botox and Dysport are neuro-modulators that treat dynamic wrinkles in the upper portion of the face. Popular uses for both of these are for the lines across the forehead, the lines between the brows (often referred to as a "scowl") and the lines on the outside of the eyes ("crows feet"). These neuro-modulators treat the cause of the wrinkle, which is the muscle beneath them that is creating the crease (or wrinkle). Botox or Dysport relaxes the muscle under the wrinkle and the skin smoothes out.

Dermal fillers, on the other hand, provide volume to the face - most commonly used on the lower portion of the face - in places where volume has been lost. As a person ages, volume may be lost under the eyes (causing the under eye dark circles to look darker and the eyes to look sunken) and in the cheekbone area. Deep creases often develop from the side of the nose to the lips (nasiolabial folds) and even on either side of the lips. With the enhancement of dermal fillers, a person may regain the face that they had five or ten years ago, giving them a more alert, rested look.

Several brands of dermal fillers are available at Evolutions. The ones that were highlighted yesterday were Restylane and Perlane. These are made from hyaluronic acid, a complex sugar molecule, that is naturally produced in the body to help provide fullness and elasticity. Restylane is a smaller molecule and better for fine lines, lasting about 6 to 8 months. Perlane is made up of bigger Restylane molecules and typically used for deeper folds. Since the Perlane molecules are bigger, it takes the body longer to break them down and Perlane may last anywhere from 6 to 9 months.

A dermal filler treatment gives a person instantaneous results! Depending the depth of the creases that are being filled (and the number of areas being treated), a treatment may use up to three syringes (3 mL) of filler to achieve "full correction". "Full correction" means using just the right amount of filler in the treatment areas of the face to get the results you want. How many syringes you will need is determined by Dr. Perkins (or the nurse) who is administering the treatment. Although you may feel that you do not want to purchase two or three syringes for "full correction", doing this proves to be much more cost effective over time. A study by Medicis showed that if a person had "full correction" and came in for a follow-up treatment before all the dermal filler had broken down (typically between 4.5 and 9 months), they ended up getting half as much the second time because when layering hyaluronic acid it binds to itself.

What is Pigmentation?

Melanin in the skin causes pigmentation - that varies by an individual's skin color (darker skin has more melanin, lighter skin has less).

Melanin is produced for protection in response to sun exposure (for example, a sun tan is really an "injury" to the skin), trauma, inflammation.

Melanin can be influenced by hormones, birth control pills and pregnancy (the "mask of pregnancy" often appears as melasma, darker spots, on the cheeks).

What if Melanocytes are Over-Stimulated?

Appearance problems of the skin occur when melanocytes are over-stimulated. This hyperpigmentation is commonly referred to as brown spots, age spots, liver spots, and/or sun damage.

Post-inflammatory hyperpigmentation can also occur from pimples.

Melasma - a broad patch of pigmentation - can occur and can be in the epidermal or dermal layers of the skin.

Epidermal pigmentation is more readily treated because of the location - the outer-most layer of the skin. Dermal region pigmentation can be more stubborn to treat. At Evolutions, we use a Wood's Lamp - a diagnostic dermatology tool - to look at a client's hyperpigmentation location to find a treatment best suited for them.

Hyperpigmentation Prevention: Use Your Sunblock Daily!

Before we address pigmentation correction treatment options, we want to stress that prevention is key! Santa Barbara cosmetic surgeon Dr. Terry Perkins recommends using a sunblock that is at least SPF 30 and is a physical block - one that contains titanium dioxide and/or zinc oxide. Physical sunblocks reflect the sun's energy whereas chemical sunscreens heat the skin up.

Dr. Perkins and the Evolutions staff recommend to clients a highly effective sunblock called TIZO3 SPF 40. TIZO is free of paraben, oil, PABA and fragrance. It gives the skin superb protection while leaving the skin with a silky, smooth, matte finish. TIZO products are available at Evolutions.

Hyperpigmentation Treatment Options

Home Care: There are many skin lightening agents on the market that are available at your local drugstore or through a physician's office. Dr. Perkins recommends the Obagi Nu-Derm or Obagi-C Rx programs, PCA Brightening Therapy, NIA 24, and retinoids (also known as "Retin A" or "tretinoin"). Another home care option, maybe in addition to a skin brightening/lightening agent, is to increase cell turnover through products with alpha hydroxy acids (AHA), amino fruit acids, and, again, retinoids.

Chemical Peels: a chemical peel is a "controlled" injury to the skin that creates enough injury to cause an exfoliation. Evolutions offers chemical peels in different strengths, depending on your overall goal and the amount of downtime you may have to dedicate to the process. Read more about chemical peels, what they can do for you, and the types that are offered at Evolutions in a previous blog post dedicated to them.

Vibraderm: a superficial exfoliation using abrasive aluminum paddles that provides a gentle abrasion that is faster than microdermabrasion and does not have a vacuum component (which can cause broken capillaries). Great in a series or with in conjunction with other procedures as it will improve the penetration of treatment products and chemical peels.

FotoFacial RF™: an Intense Pulsed Light (IPL) treatment with a broad spectrum of light that gives the FotoFacial RF™ the ability to treat different parts of the skin as the light is absorbed at different levels. The FotoFacial RF™ significantly improves your skin texture and appearance by reducing redness or rosacea, age spots, sun damage, spider veins, fine lines, uneven pigmentation and large pores. It is not a treatment for wrinkles. Your skin may be pink on the day of treatment but other than that, there is little down time.

Sublative Rejuvenation: the newest fractional resurfacing device on the market that uses radiofrequency and can treat any skin color. It can treat mild to moderate wrinkles, brown spots, and can improve the appearance of acne scars and textural irregularities. There is less discomfort with this newer treatment in comparison to the older Matrix RF treatment and the down time is not as long.

Santa Barbara Laser Hair Restoration: Recap of Evolutions Medical Spa's Educational Event

Laser hair restoration therapy is now available in Santa Barbara! This non-invasive, non-surgical hair restoration treatment was introduced by Santa Barbara cosmetic surgeon Dr. Terry J. Perkins at Evolutions Medical Spa's Educational Event on Wednesday evening.

Hair loss, in both men and women, can make someone feel very self-conscious. There are many myths as to why we lose our hair and those include overuse of shampoo and hair products, too much blow drying, stress, dying hair, wearing hats and more. The truth is that 95% of hair loss is an inherited genetic condition (and not just from your Mother's side!).

Hair grows in a cycles. A perfect hair growing cycle would be growth for about one thousand days, "resting" for about 100 days, after which it falls at. For some people these cycles turn abnormal, and that is when hair thinning (and loss) occur. In the abnormal cycle, the hair follicle becomes miniaturized, producing hair that is weak, thin and luster-less. As miniaturized follicles take over, the thinning hair does not adequately cover the head, causing the person to have balding spots (more often occurring at the crown of the head).

People cope with hair thinning and hair loss in different ways. Some try to cover it up with wigs, toupees, hats or creative hairstyles ("comb over") while others opt for surgery (hair transplant) and/or drugs (topical or oral) to stimulate hair growth. In between treatments at Evolutions Medical Spa, we stress the importance of skin care and developing an at-home skin care routine to maintain your skin. By doing this, your medical spa procedures are more effective and your overall skin health is boosted.

Our guest author, a Santa Barbara skin care expert, will be addressing skin care for women in their 30s in the article below. This is when many of our clients really begin to "take notice" of their skin, the products they are (and aren't!) using and start to focus on how to keep their skin beautiful for the rest of their adult lives.

- Skin Care Tips for Women in their 30s -

As women enter their 30s, they begin to be more conscientious of their skin care routine, perhaps because this is when sun damage, skin laxity and environmental damage to the the skin begins to show. For most women in their 20s, the skin cell turnover is still rapid (every28 days) which means their skin is constantly replenishing and repairing and the result is healthy, volume-filled, glowing skin. As women transition into their 30s, some begin to see their skin quality diminish as skin cell turnover slows (to as much as every 40 days!).

The good news is that we can keep the skin young and address environmental damage by developing a skin care routine that is appropriate for a women in her 30s. These skin care tips will keep your skin in your 20s while you move into your 30s.

Are you washing your face or are you cleansing? Many people speed through the cleansing routine but do not realize that this step sets the foundation for your skin. If you want products (make up, moisturizer, sunscreen, retinoid cream) to penetrate evenly and effectively, then you may want to take a closer look at your morning and night time face washing routine. One of the products that I highly recommend to invest in (also the #1 recommendation of Evolutions Medical Spa and Dr. Perkins) is the Clarisonic face brush. In a short period of time, this facial brush removes six times more make up and two times more dirt and oil than cleansing with your hands alone. In addition to that, the sonic frequency stimulates the skin, encouraging collagen production (another plus for more mature skin). The Clarisonic can be used with any cleanser: if you have oily or combo skin, use a gel or foaming cleanser and if you have normal or dry skin, use a mild cream cleanser.
Make an effort to prevent further sun damage. Use a daily broad spectrum sunscreen of SPF 30 or more. Even better, use a sunBLOCK that physically blocks the rays such as titanium dioxide, micronized zinc oxide or iron oxide. These reflect the UV radiation instead of chemical sunscreens that absorb UV. Sunscreen is not for just after your shower and under your make up! If you are going outdoors for a run in the morning or evening, dropping your kids at school or heading to the gym, put sunscreen on first thing in the morning after your facial cleansing - and reapply after a shower, a sweaty workout, swimming, etc., The skin on the face is delicate and needs to be protected at all times! Sun damage leads to hyperpigmentation (think "liver spots") and wrinkles - essentially it "ages" the skin, and that can happen rapidly.
Treat your skin inside and out - Antioxidize! Kudos to you if you are eating your fill of antioxidants such as berries and other foods, and even better if you use a moisturizer packed with antioxidants. This way you are treating the skin from the inside and the outside! There are some amazing skin care products that are a long way from our Grandmother's "Ponds" cream. A few favorites are: Revaleskin which features CoffeeBerry extract - a super antioxidant, the Obagi-C RX System, with 10% L-ascorbic acid, also known as Vitamin C, whose effectiveness against wrinkles and fine lines is backed by a fair amount of scientific evidence, and AFA skin care products that utilize a technology based on the acidification of amino acids that make up the skin's natural moisture system and results in effective rejuvenation.
Retin A (Tretinoin) is not just for acne. Consider using a tretinoin cream two to three times per week. Derived from Vitamin A, it penetrates the skin and helps to repair sun damage as well as effectively easing the appearance of fine lines and wrinkles by increasing skin cell turnover rate and boosting the synthesis of collagen. Tretinoin is available by prescription from a physician.

Often times a reputable medical spa can customize a skin care program that encompasses some, or all, of the above products that are suitable for your skin type. There are also medically non-invasive procedures that you can do from time to time at a medical spa, or with a cosmetic surgeon, to boost your at-home skin care. Some examples follow:

Owner and Medical Director, Santa Barbara cosmetic surgeon Dr. Terry Perkins, and the staff of Evolutions welcomed guests at 4:00pm and were surprised to find a line down the street of people waiting before that! Enjoy skin care tips and suggestions for women in their 50s and beyond from our in-house skin care expert at Evolutions Medical Spa Santa Barbara.

Last week at the November Evolutions Event, the featured product was Radiesse, a dermal filler, designed to add volume to faces that have aged. The representative from Radiesse highlighted several reasons about why we age, including collagen loss. After sitting through the presentation, I had two thoughts - the first being that we are lucky to live in this day and super science-savvy age where the availability of products to "reverse" (or at least "freeze") the signs of aging are in abundance and, second, that an article about skin care tips for women in their 50s and beyond would be a great follow-up to the Radiesse presentation.

As I mentioned before, the decline of collagen and elastin are one of the primary reasons that the face shows signs of aging and this can happen as young as 25. Through research, we have found that there are ways to stimulate the collagen, which helps to maintain younger, healthy-looking skin. Following these tips may not turn back the clock (for everyone), but certainly will give your skin a boost:

Cleanse! Cleanse twice daily with a mild, rich and lathering foam cleanser.

Protect your skin. Sun damage is another culprit in aging your skin and can often create uneven pigmentation as well. Use an SPF 30 (or higher) broad spectrum sunblock and one that creates a physical barrier is even better. A physical sunblock sits atop your skin and must have these two active ingredients: zinc oxide and titanium dioxide (TIZO SPF 40).

Moisturize with antioxidants (Revaleskin with CoffeeBerry extract) and or Pro-Niacian (Nia 24). Use on the face, hands, décolleté, and neck.

Use a retinoid (such as Retin-A) two times per week. A retinoid helps to stop the breakdown of collagen after sun exposure and can increase the amount of new collagen formed (read more about this in an article from the NY Times).

Treat yourself to a facial . . . on a regular basis (every 4 to 6 weeks is optimum). Aside from giving you an hour or more of alone time where you can relax and feel pampered, a comprehensive facial can clean and exfoliate your skin, moisturize, re-balance and re-hydrate your skin. All these factors encourage collagen production so that your facial "glow" may last longer than the day your receive the treatment!

The next set of tips are for those of you who want some "help" in truly turning back the clock! All of these procedures are non-invasive and can be done at Evolutions Medical Spa by cosmetic surgeon Dr. Perkins or one of his well-trained aesthetic nurses.

Chemical peels and/or FotoFacials address skin irregularities and sun damage. Chemical peels are a great way to see results rapidly. FotoFacials are usually done in a series for best results.

The face, as we age, can be compared to a deflating balloon. What you need to know is that it can be refilled - or volumized - with Radiesse, an injectable dermal filler.

What is Radiesse?

Is Radiesse safe?

Radiesse has been FDA-approved for cosmetic use since December 2006. Over 2 million syringes have been sold! Previous to 2006, Radiesse was used for medical reasons including as a filler for cranial facial defects, for patients with facial lipoatrophy and to address issues with vocal cords.

Do the injections hurt?

Radiesse has worked to enhance the patient's comfort level by mixing lidocaine in with the product. Patients report that it has reduced the pain by 60% (and the reported pain level dropped from a 6.8 to a 2.8 out of 10). Also, many physicians, including Evolutions Medical Spa, will apply a topical anesthetic for additional patient comfort.

Other facts about Radiesse

There is little to no downtime after injections

There is no allergy testing because no one is allergic to the components in it

It is purchased by syringe (1.5cc is a full syringe) which often can provide full correction (but depends on the patient)

Do not use while pregnant or breastfeeding

On the day that the injections are done, it is recommended to refrain from exercising (to prevent bruising) and suntanning

Some of the more common side effects are:

Last week, Evolutions Medical Spa and Santa Barbara cosmetic surgeon Dr. Terry Perkins hosted an event that is a popular topic amongst many of our clients - skin tightening and skin lightening using advanced technology that gives results, without surgery. Read on for a full recap:

Part 1: Skin Tightening - Treating Skin Laxity

What is Skin Laxity?

Skin laxity, or "loose" skin, begins as a natural part of aging, due to damage and loss of collagen and elastin, but also related to sun damage.

Treating Skin Laxity

There are surgical options to successfully treat skin laxity such as:

Traditional Cosmetic Surgery: face lifts, eye lifts, neck lifts and the like.

Deep Peels: Chemabrasion, a deep peel that superficially destroys the epidermis and dermis, done under general anesthesia.

There are also successful non-surgical treatments:

Light-based treatment: such as the FotoFacial RF (photo rejuvenation), the safest and most effective intense pulsed light (IPL)-based treatment today.

Radiofrequency treatment: such as Sublative Rejuvenation, the newest skin resurfacing device with less downtime and discomfort.

How Do We Treat Skin Laxity Non-Surgically?

We have found that sustained heating of the dermis (the cells below the skin and the epidermis) has both short term and long term effects: Immediately it stimulates collagen contraction, which helps to tighten the skin while it also promotes long term stimulation of collagen remodeling - maintaining a strong collagen "support system" gives the skin strength and resilience.

Who Can Be Treated Non-Surgically for Skin Laxity?

Clients that have the best results have a minimum amount of excess fat in the area of concern and who have skin that is neither very thin or very thick.

Clients can be anywhere from 30 to 80+ years.

When treating skin laxity non-surgically, clients must acknowledge that the treatments will turn back the clock a few years, not decades!

Someone who is not ready for surgery, but ready to see a difference in their skin, is a great candidate.

The Titan Procedure: A Non-Surgical Skin Laxity Treatment

Titan uses an infrared light source to heat the dermis well below the skin's surface. This heating contracts existing collagen and stimulates growth of new collagen. Although a single treatment may show a change in your skin, a series of two to four treatments is recommended for optimum results. Downtime is minimal, with maybe some mild swelling and redness for a few hours post-treatment. Clients have been successfully treated on many areas of the body. The most common areas treated are the jawline, under the chin, and the abdomen.

ReFirme™: A Non-Surgical Skin Laxity Treatment

ReFirme™ combines Bi-Polar Radio Frequency and Light energies to heat the dermal tissue within the targeted treatment area. Part 2: Skin Lightening - Improving Excess Pigmentation

What is Pigmentation?

Pigmentation is melanin in the skin. Melanin is produced by melanocytes and different skin types have differing levels of melanocyte activity.

Why Does the Body Produce Melanin?

Every body has a normal production of melanin. The body also produces melanin in response to sun exposure (suntan = skin injury), trauma, and inflammation. Melanin production can be accentuated by hormones, birth control pills, and pregnancy.

Pigmentation Problems

If melanocytes are over stimulated, appearance problems can occur and can be commonly referred to as sun spots, age spots, "liver" spots, and freckles.
Post Inflammatory Hyperpigmentation (PIH) can cause darkening and discoloration as a result of an inflammatory reaction, or injury, to the skin. Common causes are scratches, burns, cuts, bruises or even certain rashes.
Melasma, darkening patches of the skin, usually appears on the face (although it can develop on the forearms and neck) and can be triggered by pregnancy, birth control pills, photosensitive drugs, sun exposure.

Treatment for Hyper Pigmentation

Sunscreen: Proactively protect your facial and chest skin year round using a good sun block every day.

Home Care: There are products that can be used at home to fade pigmentation (bleaching agents) and increase cell turnover.

Bleaching agents: Recommended bleaching agents are the Obagi Clear, which is Step 3 in the Obagi Nu-Derm system, Obagi C-RX Night Cream, both contain Hydroquinone, PCA Brightening Therapy, Nia24 products and retinoids.
Increase cell turnover: Alpha-Hydroxy Acids, Amino Fruit Acids and Retinoids all play a roll in expediting cell turnover.

Chemical Peels: Chemical exfoliation, which is what happens when a chemical peel is done, helps to achieve / maintain clear and youthful skin, especially because as we age, the natural exfoliation process tends to slow down, leaving skin dry and dull. Evolutions offers three types of chemical peels:

Phase Five Peel (light peel): low irritation and not a lot of actual peeling - a series of five is recommended.
Alpha Beta Peel (medium peel): a more aggressive medium-depth peel that stimulates new collagen and elastin. Clients usually see results in as little as four to five days.
Obagi Blue Peel (deep peel): the most aggressive peel that will significantly improve the complexion, leaving the skin firmer, clearer, smoother and healthier. Particularly good for pigmentation issues too.

Vibradermabrasion: a physical exfoliation to stimulate collagen. Vibraderm keeps your skin looking young and can be used to treat superficial pigmentation problems (with no downtime) and, in addition, improves penetration of products / peels. It is superior to microdermabrasion because there is no suction, which can cause broken capillaries.

Light-based Treatments: The FotoFacial RF is the "gold standard" for treatment of pigmentation. It works deeper than most chemical peels and is available in a series or combination with other treatments (FotoPeel, FotoPeel Plus, and Foto Genesis Packages).

Sublative Rejuvenation: The newest fractional resurfacing device available that is used to treat wrinkles, pigment, acne scarring, skin laxity, tone and texture that uses radiofrequency energy instead of light, so it is safe for all skin types. Less downtime and discomfort than older fractional laser devices, with customizable treatments available. Sublative Rejuvenation can be combined with other light-based treatments such as the FotoFacial RF or Total Tightening.

Chemabrasion: a surgical procedure that combines a deep peel with dermabrasion with approximately a one-week downtime. Chemabrasion treats wrinkles, sun damage and pigmentation issues.

In conclusion, Dr. Perkins stressed that the best way to treat sun damage is to prevent it - use sunscreen daily, year round. When pigment issues arise, however, there are a number of treatment options - from "at home" care to treatments at Evolutions.

What is Rosacea?
Rosacea is not acne. It is a skin condition that produces facial redness and blemishes, similar to acne. How do we know the difference between acne and rosacea? Rosacea has no underlying bacterial growth, whereas acne does. Like acne, there is no "cure" but there are ways to maintain rosacea.

Facts About Rosacea

Rosacea is a common skin disorder affecting over 13 million Americans

You can be genetically predisposed towards Rosacea and it usually affects fair-skinned people

There are four stages/subtypes of Rosacea
Rosacea typcially starts with people in their 20s and 30s at "Stage 1" - redness in the cheeks - and peaks a person's 40s or 50s

Rosacea develops slowly over the years

Women are affected 3 to 1 versus men
Rosacea can be characterized by flare-ups and remissions and also can be "triggered" (triggers discussed later in the post)

Stages/Subtypes of Rosacea

Facial Redness: flushing and persistent redness; visible blood vessels may also appear

Bumps and Pimples: persist ant facial redness with bumps or pimples, often seen following subtype 1

Skin Thickening: Skin thickening and enlargement, usually around the nose (Dr. Perkins showed pictures of W.C. Fields as an example of thickening around the nose)

Eye Irritation: Watery or bloodshot appearance, irritation, burning or stinging

Causes of Rosacea
Exact cause(s) are unknown, but there are several theories:

Factors related to blood flow

Skin bacteria

Microscopic skin mites

Irritation of hair follicles

Sun damage of connective tissue

Abnormal immune / inflammatory response

Rosacea Triggers

Weather
Emotional influence

Physical exertion

Temperature-related activity

Food and Beverages

Medications and Topical Skin Care Products

Treatment and Management of Rosacea
As mentioned earlier, there is no "cure" for Rosacea but there are ways to manage the condition such as:

Avoid trigger factors

Gentle skin cleansing

Therapeutic home care: Dr. Perkins recommends the Obagi Rosaclear System that is the first, and only, complete system to effectively meet the needs of rosacea patients (the Rosaclear System is available at Evolutions)

Sun protection

FotoFacial RF: the safest and most effective intense pulsed light (IPL) therapy treatment today, the FotoFacial improves skin texture and appearance through reducing redness in Rosacea. At Evolutions, we have seen clients who have been through a series of FotoFacial treatments where the Rosacea has almost completely gone away.

Laser Genesis Skin Therapy: often referred to as a "laser facial", the Laser Genesis stimulates collagen growth and reduces redness by gently heating the upper dermis, another tool to use against Rosacea. Dr. Perkins highly recommends a FotoFacial / Laser Genesis combination to effectively put Rosacea into remission.

Antibiotics: tetracycline antibiotics can help quiet Rosacea.

Surgery: this can be done to tackle the skin thickening that sometimes occurs in later stage patients. Usually the thickening is around the nose and surgery will peel back the layers to bring the nose back to more of it's original form.

Acne Facts

Acne is one of the most common of all skin problems

Acne affects over 70 million people and 90% of all teens

Acne is an ongoing condition, often characterized by periodic flare-ups

It most commonly affects the face, upper chest and back

For most people it is a matter of "controlling" acne instead of curing it.

Types of Acne

Comedonal Acne: this type of acne usually consists of plugged pores with not a lot of bacterial activity, often seen as small bumps under the skin.

Papulopustular Acne: this type of acne has more bacterial activity and appears more as pimples with a whitehead and red surrounding area.

Cystic Acne: this type of acne is more deeply rooted, with the acne appearing as large bumps (like a marble under the skin) with, or without, whiteheads and may need to be treated systemically (with oral antibiotics or Accutane).

What Causes Acne?

Increased sebum or oil production - usually related to hormones

Obstruction of the pore

P. Acnes bacteria begin to grow in the sebum

Inflammation occurs when the sebum escapes into the surrounding skin

Treating Acne
Treating acne is an ongoing process because there is no "cure" your goal is to "control" what is going on. There are three overall goals:

Prevent the blocking of the pores. Opening pores helps the skin function better. Keep the pores open through exfoliation, which promotes skin turnover. Use of retinoids (like Retin-A) and exfoliation through products are recommended for at-home skin care. It is also recommended to seek periodic professional treatments such as a chemical peel or Vibradermabrasion (a more physical way to open up the surface) to expedite exfoliation of the top layers of skin.

Minimize inflammation. Salicyclic acid liquefies the sebum and helps to quiet the skin. A low percentage of salicyclic acid is okay to use on a regular basis and can help minimize the inflammation of acne.

Prevent Scarring. Acne scarring or pitting remains long after the acne is gone which is why it is a good idea to learn how to get your acne under control.

Tips on the Treatment of Acne

Good hygiene: keep the skin clean. Dr. Perkins does not recommend washing the face more than two times per day. Washing the face more than that may cause an increase in oil production. Use a gentle foaming cleanser. The face wash does not need to be harsh or it may "strip" your skin - again causing increased oil production - especially if you are using other products such as salicyclic acid or retinoids. The Clarisonic skin care brush is highly recommended by Dr. Perkins. He touts this as the "best cleaning device" because the ultrasonic waves penetrate below the surface for a deep, yet gentle, cleanse.

Don't "pick" and "pop": this may increase the life of the pimple, making it larger or redder and /or increase your chances of scarring.

No oil based cosmetics: many store make-up brands are petroleum based, which is like putting putty in your pores. Over time, this clogs pores. Mineral make-up is more quieting to the skin.

Don't go to sleep with make-up on: give your face a rest!

"Good" diet: consuming foods with a high iodine content (salt) tends to foster breakouts. In addition, a skin supplement that has worked for some Evolutions clients are the VitaMedica "Clear Skin Treatment".

Stress management: Learn to manage your stress. Outbreaks can definitely be stress (and hormone) related.

How You Can Treat Your Acne

Treatment of acne can be done in many ways, from at-home topical treatments to professional treatments to oral medication. How acne is treated typically depends on your specific case of acne and it is recommended that you get a professional opinion on how to best proceed in managing your acne.

Topical Acne Treatments

Retinoids (Retin-A), exfoliants (AFA Exfoliating Gel), and amino fruit acids can be used topically to treat acne.

Although glycolic acid is very popular and found in many store products, it can be the most irritating of the alpha hyrdoxy acids and Dr. Perkins prefers mandelic acid (available at Evolutions) which is less irritating and has an anti-bacterial function to decrease the P. Acnes bacteria.

Oral / Systemic Therapy

Oral treatments include antibiotics and Accutane. Dr. Perkins prefers to try to treat the acne topically before prescribing an oral treatment.

Professional Procedures

Acne Facials professionally cleanse, exfoliate and extract keeping the pores open and the skin functioning normally.

Vibradermabrasion is a physical exfoliation that is similar to microdermabrasion without the suction. It has been found that the suction in microdermabrasion may affect the tiny blood vessels in the face, making the skin appear redder. Vibraderm is similar to "buffing" the skin, is comfortable, and the whole face can be done in 10 to 15 minutes as opposed to a 35 to 40 minute session of microdermabrasion.

AFA/Clay Mask Peel is an advanced amino fruit acid mask which exfoliates and opens up the pores. It is less irritating than alpha hydroxy acids.

Multiwave LED (Light Emitting Diode) therapy works in which energy is delivered to the skin and absorbed by the cells for therapeutic benefits through a process called “photo-stimulation.” The "blue" light kills the P. Acnes bacteria while the "red" light calms the skin and gets rid of inflammation. The Multiwave LED therapy can be done alone or often is part of a facial at Evolutions.

Levulan is similar to the oral drug Accutane, but is a topical treatment. Levulan causes the sebaceous cells to sink and people have seen dramatic changes in their skin from one treatment or through a series (which is often three treatments).

Cyst Injections are a topical way to treat cystic acne. Antibiotics are injected under the cystic acne to help bring done the bacterial infection.

Isolaz Deep Pore Therapy combines a vacuum with a light treatment. The vacuum purifies the pores from the inside out and the blue light kills the bacteria. A great treatment for persistent papulopustular acne.

How do chemical peels work and what do they do?

A chemical peel is a controlled-depth injury to the outer layers of the skin. The "injured" tissue exfoliates ("peels") and new cells rapidly replace the exfoliated cells while collagen and elastin production is increased. Collagen works with elastin to support the body's tissue - including your facial tissue - keeping your body firm and strong.
